Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ginkgo interacts with.

Theoretically, ginkgo might increase levels of drugs metabolized by CYP1A2.
Laboratory research suggests that ginkgo leaf extract can mildly inhibit CYP1A2 enzymes. However, clinical research suggests ginkgo might not affect CYP1A2. Until more is known, use ginkgo cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for bendamustine (Treanda, Belrapzo, Vivimusta). Your liver uses an enzyme called CYP1A2 as one of the ways it clears this chemotherapy drug. Lab studies hint that ginkgo can mildly slow that enzyme down. In theory, that could let bendamustine build up a little more than expected, which might mean stronger side effects like low blood counts (raising infection, bleeding, and fatigue risk), nausea, or fever.

I want to be honest, though: this is mostly a theoretical, lab-based concern, and actual human studies suggest ginkgo may not meaningfully affect CYP1A2. Still, bendamustine is serious chemotherapy with little room for error, and ginkgo can also thin the blood. Please don't start or stop anything on your own. Run it by your oncology team or pharmacist first.

What Is Bendamustine Injection Used For?

Brand names: Treanda, Belrapzo, Vivimusta

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
