CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Prednisolone

Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP3A4.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

Here's the gist of it: your body uses a liver enzyme called CYP3A4 to break down prednisolone. There's some thought that ginkgo might speed that enzyme up a little. If that happened, your body could clear prednisolone a bit faster, which in theory could make it work less well for whatever you're treating, like inflammation, arthritis flares, or an autoimmune condition.

That said, I want to keep this in perspective. The evidence here is shaky and conflicting. Studies on other CYP3A4 medications mostly showed no meaningful change from ginkgo, so a big effect on prednisolone is unlikely. Still, if you rely on prednisolone for something serious, it's worth a quick chat with your pharmacist or doctor before adding ginkgo, and don't change your steroid dose on your own.

P-GLYCOPROTEIN SUBSTRATES

Prednisolone also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, taking ginkgo with P-glycoprotein substrates might increase the levels and adverse effects of these substrates.
A small clinical study in healthy volunteers shows that using ginkgo leaf extract 120 mg orally three times daily for 14 days can increase levels of the P-glycoprotein substrate, talinolol, by 36% in healthy male individuals. However, single doses of ginkgo do not have the same effect.

Here's the gist of it. Your body uses a little pump called P-glycoprotein to help shuttle certain medicines out of your cells and gut. Prednisolone is one of the drugs that pump moves around. Ginkgo may slow that pump down a bit, which in theory could let a little more prednisolone hang around in your system.

If that happened, you might notice more of the usual steroid effects, like trouble sleeping, a bigger appetite, mood swings, fluid retention, or higher blood sugar. That said, this is mostly theoretical. The human evidence comes from a different drug, and only steady daily ginkgo use (not a single dose) nudged levels up. I wouldn't lose sleep over it, but do mention your ginkgo to your pharmacist or doctor so they can keep an eye on things.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
