CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Hydrocortisone

Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP3A4.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

When you swallow hydrocortisone pills, your body relies on an enzyme called CYP3A4 to break it down. The thinking here is that ginkgo might speed that enzyme up a bit, which could clear hydrocortisone a little faster and make it slightly less effective. In practice that might mean your steroid doesn't control symptoms like inflammation, allergic flares, or arthritis pain as well as expected.

Honestly, this is mostly theoretical. Studies on ginkgo and CYP3A4 are mixed, and several found no meaningful effect. The clearest signal came from one HIV medication, not from steroids. So I wouldn't lose sleep over it, but if you take hydrocortisone for something important like adrenal support, please run it by your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES

Hydrocortisone also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, taking ginkgo with P-glycoprotein substrates might increase the levels and adverse effects of these substrates.
A small clinical study in healthy volunteers shows that using ginkgo leaf extract 120 mg orally three times daily for 14 days can increase levels of the P-glycoprotein substrate, talinolol, by 36% in healthy male individuals. However, single doses of ginkgo do not have the same effect.

Here's the basic idea: hydrocortisone is one of many medicines moved around your body by a transporter protein called P-glycoprotein, which acts a bit like a bouncer pushing the drug back out of cells. Ginkgo may slow that bouncer down. In theory, that could let a little more hydrocortisone stick around, which might nudge up its effects and side effects (think water retention, higher blood sugar, stomach upset, or trouble sleeping).

I want to be honest about the strength of this, though. The concern is largely theoretical, and the human study that found a real bump used a different drug, not hydrocortisone. For most people this is a minor worry, but if you take hydrocortisone regularly, just give your pharmacist or doctor a heads-up before adding a daily ginkgo supplement.

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
