Why this applies to Levonorgestrel

Levonorgestrel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP3A4.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

Here's the situation with ginkgo and levonorgestrel (the active ingredient in Plan B and several other birth control products). Your body partly clears levonorgestrel using a liver enzyme called CYP3A4. In theory, ginkgo could speed that enzyme up a bit, which would clear the hormone faster and could make it slightly less effective at preventing pregnancy. Levonorgestrel works as-is, so it is not a prodrug that needs activating.

That said, the evidence here is weak and conflicting. Studies have gone both ways, and ginkgo doesn't seem to strongly affect this enzyme in the liver for most people. I wouldn't lose sleep over it, but since this is about preventing pregnancy, it's worth a quick chat with your pharmacist or doctor.

Ginkgo is one of the world's most popular herbal supplements, mostly taken to support memory and circulation. The evidence for these uses is mixed and generally weak, and it is not proven to prevent memory loss or dementia. About Levonorgestrel

What Is Levonorgestrel Used For?

Brand names: Plan B, Mirena, Levonelle One Step, Levonelle 1500, Norgeston, Skyla, Liletta, Kyleena

Levonorgestrel is used in two very different ways depending on the product. As an intrauterine system (IUS), Mirena and Liletta prevent pregnancy for up to 8 years and also treat heavy menstrual bleeding for up to 5 years in people who choose intrauterine contraception. Kyleena prevents pregnancy for up to 5 years, and Skyla prevents pregnancy for up to 3 years — neither Kyleena nor Skyla is indicated for heavy menstrual bleeding.

As an oral tablet, products such as AfterPill, AfterPlan, Amazon Basic Care Levonorgestrel, Econ Morning After, Emergency Contraceptive, and Empowered Choice are used as emergency (backup) contraception to reduce the chance of pregnancy after unprotected sex or contraceptive failure. The oral tablet is not intended for routine birth control.
