ANTIHYPERTENSIVE DRUGS

Irbesartan falls under ANTIHYPERTENSIVE DRUGS — the group Rhodiola interacts with.
In vitro and animal research shows that rhodiola extract inhibits angiotensin-converting enzyme (ACE) and might lower blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Irbesartan (Avapro) already does the work of lowering your blood pressure by relaxing your blood vessels. Rhodiola, in lab and animal studies, seems to have its own mild blood pressure lowering effect. So the worry is that stacking the two could push your pressure a little lower than intended.
If that happened, you might feel dizzy, lightheaded, or tired, especially when you stand up quickly. I'll be honest though: this concern comes only from test tube and animal research, not from people, so it's theoretical and fairly weak evidence. Irbesartan is not a prodrug, so this isn't about how your body activates it. Still, it's worth a quick chat with your pharmacist or doctor before adding Rhodiola.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
2 of 2Irbesartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP2C9 to help clear irbesartan. Rhodiola may slow that enzyme down a bit. If irbesartan is broken down more slowly, a little more of it can hang around, which could nudge its blood-pressure-lowering effect up. In real life that might show up as feeling lightheaded or dizzy, especially when you stand up quickly, or being more tired than usual.
I want to keep this in perspective, though. The evidence here is mostly lab-based plus one small study in a similar drug (losartan), and the effect was modest and varied a lot by product. Irbesartan isn't a prodrug, so this points toward a slightly stronger effect, not a weaker one. If you'd like to try Rhodiola, just loop in your pharmacist or doctor first so they can keep an eye on your blood pressure.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. What Is Irbesartan Used For?

Irbesartan tablets — sold as the brand Avapro and as generic irbesartan — are used to treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of serious events like stroke and heart attack. It can be used alone or alongside other blood pressure medicines.
Irbesartan tablets (including Avapro) are also used to treat diabetic nephropathy — kidney disease caused by diabetes — in people who have type 2 diabetes, high blood pressure, and signs of kidney damage. In this group, it helps slow how quickly the kidney disease gets worse.
