Why this applies to Alpelisib
Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for alpelisib (Piqray): your body relies on a liver enzyme called CYP3A4 to help clear it. In the test tube, rhodiola can slow that enzyme down a bit. If that happened in real life, alpelisib levels might creep up, which could mean more of its usual side effects like high blood sugar, rash, diarrhea, nausea, or mouth sores.
The good news is this is mostly theoretical. The effect varies a lot between rhodiola products, and a human study found rhodiola did not actually slow the clearance of a similar CYP3A4 drug. Alpelisib is not a prodrug, so it doesn't flip the other way. Still, since this is a cancer medicine, please run any rhodiola use past your oncologist or pharmacist first.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.
About Alpelisib
What Is Alpelisib Used For?
Brand name: Piqray
Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.
Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
