Why this applies to Tadalafil
Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the quick version with Rhodiola and Tadalafil (Cialis). Tadalafil is cleared from your body by a liver enzyme called CYP3A4. In a test tube, Rhodiola can slow that enzyme down, which in theory could let more tadalafil build up and lead to stronger side effects like headache, flushing, a stuffy nose, back pain, or lightheadedness.
The good news: this is mostly lab-based and shaky. A real human study found that Rhodiola did not meaningfully change how the body handles a similar CYP3A4 drug, and this interaction is rated as minor. Tadalafil is active on its own (not a prodrug), so the worry would be a bit too much, not too little. Still, since both can lower blood pressure a touch, check with your pharmacist or doctor before combining them.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. About Tadalafil
What Is Tadalafil Used For?
Brand names: Cialis, Tadliq
Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.
Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
