Why this applies to Tadalafil

Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice can increase levels of drugs metabolized by CYP3A4.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.

Here's the deal with grapefruit and your tadalafil (Cialis). Your liver uses an enzyme called CYP3A4 to clear tadalafil out of your system. Grapefruit slows that enzyme down, so the drug hangs around longer and your blood levels climb higher than expected. Tadalafil is active as-is (not a prodrug), so more of it in your body means a stronger effect.

In practice that can mean more of tadalafil's usual side effects: headache, flushing, a stuffy nose, back pain, or dizziness, and a bigger drop in blood pressure (especially worrisome if you also take nitrates or blood pressure pills). This effect can linger a day or two after the grapefruit. An occasional bite probably isn't a crisis, but regular juice or whole fruit adds up. Please run it by your pharmacist or doctor before mixing the two.

Here's the gist for tadalafil (Cialis): your liver clears it mainly through an enzyme called CYP3A4. Grapefruit can slow that enzyme down, so the medicine may hang around longer and climb a bit higher in your blood than usual. In plain terms, that could nudge tadalafil's effects and side effects up, things like headache, flushing, a stuffy nose, indigestion, back pain, or feeling lightheaded if your blood pressure dips.

I'd keep this in perspective, though. The concern comes from animal and lab studies, not solid human data, so it's more theoretical than proven. An occasional grapefruit probably isn't a big deal, but if you eat it often or drink the juice regularly, it's worth a quick chat with your pharmacist or doctor before changing anything.

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
