Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Here's the situation with Syrian Rue and your modafinil. Your body partly clears modafinil using a liver enzyme called CYP3A4. The harmala alkaloids in Syrian Rue (harmine, harmane, and friends) can slow that enzyme down. In theory, that means modafinil could clear a bit more slowly and build up in your system. Modafinil is an active drug, not a prodrug, so more of it sticking around usually means stronger effects, things like headache, jitteriness, a racing heart, trouble sleeping, nausea, or feeling anxious.

That said, this is a theoretical concern based only on lab studies, not real patients, so we don't know how big a deal it is in practice. Syrian Rue also carries its own toxicity risks. Please check with your pharmacist or doctor before combining them.

Theoretically, Syrian rue might increase the adverse effects of dopamine agonists.
Syrian rue contains the constituent harmine. Animal research shows that harmine increases extracellular dopamine levels. The clinical significance of this effect is unclear.

Here's the gist with modafinil and Syrian Rue. Modafinil is a wakefulness-promoting medicine that nudges up brain chemicals like dopamine. Syrian Rue's harmala alkaloids (mainly harmine) appear, at least in animal studies, to raise dopamine levels too. So in theory, stacking them could push dopamine activity higher than intended.

What that might feel like is more of modafinil's own stimulant-type effects: a racing or pounding heart, jitteriness, anxiety, trouble sleeping, headache, or higher blood pressure. I want to be honest though, this is a theoretical concern based on lab and animal data, not proven in people, so the real-world risk is unclear. Syrian Rue also acts as an MAO inhibitor and carries its own toxicity risks. Please check with your pharmacist or doctor before combining them.

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
