Dextromethorphan falls under SEROTONERGIC DRUGS — the group Syrian Rue interacts with.
Syrian rue contains harmaline and harmine, which have demonstrated strong, reversible inhibition of monoamine oxidase (MAO) A and weak inhibition of MAO B in animal and in vitro research. This may increase the risk for adverse effects with serotonergic drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing these two. Dextromethorphan raises serotonin activity in the brain, and Syrian Rue's harmala alkaloids (harmine and harmaline) act like an MAO inhibitor, which slows the breakdown of serotonin. Stack those together and serotonin can climb too high, raising the risk of a reaction called serotonin syndrome. That can look like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion.
I'll be honest: this warning is mostly theoretical, based on lab and animal studies, not strong human data. Still, the combination of an MAOI and dextromethorphan is a well-known one to avoid. Please check with your pharmacist or doctor before using Syrian Rue, and skip it if you're taking a dextromethorphan cough product.

Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing about your cough medicine: your body relies on an enzyme called CYP2D6 to break down dextromethorphan and clear it out. Syrian Rue's harmala alkaloids appear to slow that same enzyme down. If the cleanup crew is working slower, dextromethorphan can build up higher than usual, which may mean more drowsiness, dizziness, confusion, or feeling foggy and unsteady.
There's an extra wrinkle worth knowing. Syrian Rue also acts like an MAO inhibitor, and combining MAOIs with dextromethorphan can rarely trigger something called serotonin syndrome (agitation, fever, racing heart). For now this is mostly theoretical, based on lab studies, not strong human data, but I wouldn't pair these. Please check with your pharmacist or doctor first.

Here's the gist for your cough medicine. Syrian Rue's harmala alkaloids may slow down CYP3A4, one of the liver enzymes that helps clear dextromethorphan from your body. If that enzyme is blocked, dextromethorphan could build up and hit harder than usual, with effects like drowsiness, dizziness, confusion, or feeling out of it.
There's an extra wrinkle worth flagging: Syrian Rue also acts like an MAO inhibitor, and combining MAOIs with dextromethorphan is a known concern for serotonin-related reactions (agitation, rapid heartbeat, sweating, shakiness). The CYP3A4 piece itself is theoretical, based on lab studies, not people. Still, given both issues, I'd really run this past your pharmacist or doctor before mixing them.

There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.
Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
