Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.

Here's the simple version: your body relies on an enzyme called CYP2D6 to clear dextromethorphan out of your system. Lab studies suggest higenamine can block that enzyme. If that holds true in people, dextromethorphan could hang around longer than usual and build up, which may mean stronger or longer-lasting effects like drowsiness, dizziness, nausea, or feeling unsteady or foggy.

I want to be honest about the evidence here. This has only been seen in test-tube research, not in actual patients, so it's more of a theoretical concern than a proven problem. Still, since higenamine itself is a heart and blood pressure stimulant with iffy labeling, I'd run this combo past your pharmacist or doctor before mixing the two.

Here's the gist for your cough medicine: higenamine may slow down one of the liver enzymes (called CYP3A4) that helps clear dextromethorphan from your body. If that enzyme works more slowly, the drug could linger a bit longer and its effects could feel stronger. With dextromethorphan, that might mean more drowsiness, dizziness, or feeling foggy or off-balance.

I want to be honest about how shaky this is, though. The only evidence comes from a lab dish, where higenamine blocked the enzyme by about 21 percent, and this has never actually been shown in real people. So I wouldn't lose sleep over it. Still, higenamine itself raises heart-rate and blood-pressure questions, so it's worth running both past your pharmacist before combining them.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
