Why this applies to Lidocaine

Lidocaine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the gist: lidocaine is partly broken down by a liver enzyme called CYP2D6. In lab tests (not in real people), higenamine appears to slow that enzyme down. If that happened in the body, more lidocaine could linger, which in theory means a bit more drug effect and a higher chance of side effects.

That said, this is theoretical and based only on test-tube data. It also matters how you use lidocaine. The creams, patches, and gels you rub on your skin (like Aspercreme, Salonpas, or Lidoderm) put very little into your bloodstream, so a meaningful interaction is unlikely. Injected or larger doses would be the bigger concern. Honestly, with higenamine's own heart and blood pressure worries, I'd run any combo past your pharmacist or doctor first.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down lidocaine, and lab studies hint that higenamine may slow that enzyme down a bit (around 21% in a test tube). If that happened in real life, lidocaine could clear a little more slowly and build up, which in theory could mean more of its effects or side effects, like dizziness, numbness or tingling, drowsiness, or in larger amounts heart rhythm changes.

The honest part is that this is only based on lab data, not on real people, so it may not matter much in practice. It's also worth noting that creams, patches, and gels you put on your skin barely reach the bloodstream, so the concern is even smaller there. Since higenamine itself can stir up your heart and blood pressure, I'd still run this combo past your pharmacist or doctor before mixing them.

What Is Lidocaine Transdermal Patch Used For?

Brand names: Xylocaine, Dilocaine, Lidoject-1, Xylocaine MPF, Nervocaine, Lidoject-2, Duo-Trach Kit, Octocaine, Lidoderm, Ztlido

Lidocaine skin patches are applied directly to the painful area to treat post-herpetic neuralgia (PHN) — the persistent burning or aching nerve pain that can remain after a shingles outbreak.

Lidoderm (lidocaine patch 5%) and ZTlido (lidocaine topical system 1.8%) are both indicated for this use in adults. Note that despite their different strengths, one ZTlido topical system provides equivalent lidocaine exposure to one Lidoderm patch.
