Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to clear nortriptyline out of your system. In lab studies, higenamine appears to slow that enzyme down. If that happened in real life, nortriptyline could build up to higher levels than expected, which may mean more of its usual side effects, things like a dry mouth, constipation, drowsiness, blurred vision, dizziness when standing, and a faster or irregular heartbeat.

I want to be honest about the evidence, though. This was only seen in a test tube, never confirmed in actual people, so it's more of a theoretical caution than a proven problem. Still, higenamine itself can stir up the heart and blood pressure, and nortriptyline can too, so I'd run this combination by your pharmacist or doctor before mixing them.
