Why this applies to Oxycodone
Oxycodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Syrian Rue interacts with.
Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing with Syrian Rue and oxycodone. Syrian Rue contains compounds that, at least in lab studies, can slow down an enzyme called CYP2D6. Your body uses CYP2D6 to convert part of your oxycodone into oxymorphone, a stronger pain-relieving byproduct. If that enzyme is blocked, that conversion shifts a bit, but oxycodone also relies on a separate enzyme (CYP3A4) for much of its breakdown, so the overall picture is mixed.
On top of that, Syrian Rue acts as an MAO inhibitor, and mixing MAOIs with opioids can be genuinely risky. The CYP2D6 part is theoretical and based only on test-tube data, but I'd still treat this combination cautiously. Watch for extra drowsiness, slowed breathing, or confusion, and please talk with your doctor or pharmacist before combining them.

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Oxycodone treats pain that is severe enough to need an opioid pain medicine, when other treatments are not enough. Other options include non-opioid pain relievers or opioid combination products.
Some products are for specific situations. Extended-release products (OxyContin, Xtampza ER, and oxycodone extended-release tablets) are for severe, persistent pain that needs daily, around-the-clock treatment. They are not for as-needed use. The 100 mg per 5 mL (20 mg/mL) oral solution is only for opioid-tolerant patients. Oxaydo is labeled for acute and chronic pain.
