Oliceridine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Oliceridine (Olinvyk) is a strong IV opioid that hospitals use for short-term, severe pain, and your liver clears it partly through an enzyme called CYP3A4. Danshen can nudge this same enzyme, but the direction depends on how long it's used. Taking danshen for a couple of weeks seems to speed up CYP3A4, which could clear oliceridine faster and make it work less well for your pain. A single recent dose can do the opposite, slowing clearance and possibly raising levels, which could mean more drowsiness, slowed breathing, or low blood pressure.

This is mostly studied with a different drug (midazolam), so it's an educated prediction, not a sure thing. Since Olinvyk is given in a monitored hospital setting, tell your care team about any danshen use so they can adjust safely.

Olinvyk (oliceridine) injection is approved for adults to manage acute pain that is severe enough to require an intravenous opioid analgesic. It is used only when alternative treatments are ineffective, not tolerated, or otherwise unable to provide adequate pain control.

Because of serious risks including addiction, misuse, overdose, and death, Olinvyk is reserved for situations where other pain management options are not sufficient. It is not intended for routine or chronic pain management.
