Why this applies to Olutasidenib

Olutasidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Olutasidenib (Rezlidihia) is your oral cancer pill for certain AML with an IDH1 mutation, and your body relies on the CYP3A4 enzyme to break it down. Danshen can nudge that enzyme in two directions, which makes this a bit tricky. Taken regularly for a week or two, Danshen tends to speed up CYP3A4, which could clear olutasidenib out faster and leave you with less of the medicine working against your cancer. A single dose, oddly, did the opposite in studies and raised levels.

Because this drug is doing serious work, I would not want it weakened or spiked unpredictably. The human evidence here is early and based on a different drug (midazolam), so it is suggestive, not certain. Please check with your oncologist or pharmacist before combining them.

It is only for patients whose leukemia cells carry a susceptible IDH1 mutation, which must be confirmed by an FDA-approved test before starting treatment.
