nefazodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your body clears nefazodone using a liver and gut enzyme called CYP3A4, and Danshen can tinker with that enzyme. Here's the tricky part: in a small study, taking Danshen for 10 to 14 days actually sped this enzyme up, which would clear nefazodone faster and could make it work less well for your depression. But a single dose of Danshen did the opposite, slowing things down so the drug builds up. That building up could mean more drowsiness, dizziness, dry mouth, or feeling lightheaded when you stand.

Nefazodone is not a prodrug, so it needs steady levels to keep your mood stable. Because the effect can swing both ways and the evidence is early, please check with your pharmacist or doctor before combining them rather than changing anything yourself.

What Is Nefazodone Used For?

Brand name: Serzone

Nefazodone Hydrochloride tablets are approved to treat depression, specifically major depressive disorder (MDD). Clinical trials established its effectiveness over 6 to 8 weeks in outpatients and 6 weeks in hospitalized patients with depression.

Nefazodone has also been shown to help prevent depression from coming back in patients who responded well to initial treatment. Because of a serious risk of liver failure, prescribers are advised to consider other antidepressants before turning to nefazodone.
