Mitapivat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body clears Mitapivat (Pyrukynd) using a liver and gut enzyme called CYP3A4, and Danshen can mess with that enzyme. Mitapivat is an active drug, not a prodrug, so the amount floating in your blood matters directly for how it works.

The tricky part is that Danshen pushes in two directions. Taken regularly for a week or two, it seems to speed up CYP3A4, which could lower your Mitapivat levels and let your anemia symptoms (tiredness, paleness, shortness of breath) creep back. A single dose may briefly raise levels instead. This is early evidence, but since this drug needs steady levels, please check with your pharmacist or doctor before combining them.

What Is Mitapivat Used For?

Brand name: Pyrukynd

Mitapivat is available as two separate branded products approved for different conditions. Pyrukynd is used to treat hemolytic anemia — a type of anemia caused by red blood cells breaking down too quickly — in adults who have pyruvate kinase (PK) deficiency, a rare inherited blood disorder. Aqvesme is used to treat anemia in adults with alpha- or beta-thalassemia, inherited conditions that affect how the body makes hemoglobin.

These are distinct approvals. Pyrukynd and Aqvesme are not interchangeable, and each is approved only for its specific indication.
