Ethinyl Estradiol, Norethindrone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for your birth control pill (the ethinyl estradiol and norethindrone combo). One way your body processes estrogen is through a liver enzyme called CYP1A2. In test-tube studies, danshen seemed to slow that enzyme down. If that happened in real life, it could let a bit more estrogen build up in your system, which in theory might mean more typical estrogen-related side effects like nausea, breast tenderness, bloating, or headaches.

The honest part: this is theoretical and based only on lab work. When danshen was tested in people with another CYP1A2 drug, it didn't actually change anything. So I wouldn't lose sleep over it, but since danshen can also affect blood clotting, please run it past your pharmacist or doctor before combining the two.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Your birth control combo (ethinyl estradiol plus norethindrone) is partly handled by a transporter called p-glycoprotein, which helps move drugs in and out of your body. Danshen may slow that transporter down. In one small study in healthy people, danshen raised blood levels of another p-glycoprotein drug by roughly a third. So in theory, taking danshen could nudge your hormone levels a bit higher.

What might that look like? Possibly more of the usual estrogen-type side effects, like nausea, breast tenderness, headaches, or bloating. This is an active hormone, not a prodrug, so it doesn't flip the direction. The evidence here is fairly limited, so don't panic, but please check with your pharmacist or doctor before combining them rather than changing anything on your own.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the practical side of this one. Your birth control pill, which combines ethinyl estradiol with norethindrone, gets processed by a liver and gut enzyme called CYP3A4. When Danshen is taken regularly for a couple of weeks, the research suggests it can speed that enzyme up, which means your body may clear the hormones faster than usual. Lower hormone levels could, in theory, make the pill less reliable, raising the risk of breakthrough bleeding or even an unintended pregnancy.

This is based on early human studies using a different drug, so it isn't a sure thing, but with contraception I'd rather you not gamble. Please check with your pharmacist or doctor before pairing Danshen with your pill so they can help you stay protected.
