Estradiol, Norethindrone Acetate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for your Activella (estradiol plus norethindrone). The estrogen part is partly broken down by a liver enzyme called CYP1A2. In lab studies, Danshen appears to slow that enzyme down. If that happened in your body, your estrogen levels could drift a bit higher, which in theory might mean more of the usual hormone-related effects like breast tenderness, bloating, nausea, headaches, or spotting.

The important part: this is theoretical. It's only been seen in test tubes, and in actual people Danshen did not change levels of another CYP1A2 medicine. So the real-world risk looks low. Still, because Danshen can also affect blood clotting, please run it by your pharmacist or doctor before combining the two.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Activella combines two active hormones, estradiol and norethindrone, so it is not a prodrug that needs activating. Part of how your body moves these hormones around involves a transporter called p-glycoprotein. In a small study, Danshen slowed this transporter down, which made another drug's blood levels climb by roughly a third. If the same thing happens with your hormone medicine, you could end up with a bit more in your system than expected.

Practically, that might mean more of the usual hormone-related effects, like breast tenderness, bloating, nausea, headaches, or breakthrough spotting. This is based on limited, lower-quality evidence, so it is a possible concern rather than a sure thing. Before adding Danshen, please check with your pharmacist or doctor.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your Activella combines two hormones, estradiol and norethindrone, and your body relies on the CYP3A4 enzyme to help break them down. Here's where Danshen comes in. When taken regularly for a couple of weeks, Danshen appears to speed up that enzyme, especially in the gut. That could push your hormone levels lower than expected, which might mean less reliable symptom control, so things like hot flashes, night sweats, or breakthrough bleeding could creep back in.

Interestingly, a single dose of Danshen seemed to do the opposite in studies, briefly raising drug levels. The research is preliminary and was done with a different medicine, so we can't be certain how it plays out with your hormones. I'd run it by your pharmacist or doctor before combining them.
