CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Ethinyl estradiol, Levonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your birth control pill, Lybrel (ethinyl estradiol and levonorgestrel). In lab tests, Danshen can slow down a liver enzyme called CYP1A2, which helps break down the estrogen part of your pill. In theory, that could let estrogen levels drift a little higher, which might mean more of the usual estrogen-related complaints like nausea, breast tenderness, headaches, or bloating.
I want to be honest, though: this is based on test-tube work only, and when researchers tested Danshen in real people with another CYP1A2 drug, they didn't see the effect. So this is a theoretical concern, not a proven one. The bigger worry with Danshen is its possible effect on blood clotting. Please run it by your pharmacist or doctor before combining the two.

P-GLYCOPROTEIN SUBSTRATES

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

If you take Lybrel (a birth control pill with ethinyl estradiol and levonorgestrel), here's the gist. Danshen can slow down a transporter in your gut called p-glycoprotein, which helps move certain medicines in and out of the body. In a small study, danshen nudged up the levels of one p-glycoprotein drug. So in theory, danshen could let a bit more of your pill stay in your system.
The good news: this is moderate, possible, and based on lower-quality research, not strong proof. Even if levels shift slightly, your contraception isn't expected to suddenly fail. You might notice estrogen-type effects like nausea, breast tenderness, or spotting if levels rise. Before adding danshen, check with your pharmacist or doctor, especially since danshen may also affect blood clotting.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the practical concern with your birth control pill (Lybrel). Both hormones in it, ethinyl estradiol and levonorgestrel, are partly broken down by a liver and gut enzyme called CYP3A4. When Danshen is taken regularly for a week or two, the research suggests it can speed that enzyme up, which means your body may clear these hormones a little faster than usual.
Why does that matter? Lower hormone levels could, in theory, make the pill less reliable, raising the small risk of breakthrough bleeding, spotting, or even reduced contraceptive protection. The evidence here is limited (it comes from a study using a different drug), so this isn't a sure thing. Still, since an unplanned pregnancy is a big deal, please check with your pharmacist or doctor before pairing them, and ask about a backup method.
