Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist: propranolol is a beta blocker that lowers blood pressure and slows the heart. Danshen, in animal studies, also seems to nudge blood pressure downward. Put the two together and, at least in theory, their blood-pressure-lowering effects could stack up. For an infant taking propranolol for a hemangioma, you'd watch for signs like unusual sleepiness, poor feeding, a slow or weak heartbeat, or cool, pale skin.

I want to be honest about how soft this evidence is. It comes from lab and animal research, not solid human studies, so it's more of a caution than a proven problem. Still, this drug is given to tiny babies, so I'd never add Danshen without first checking with your pediatrician or pharmacist.

Propranolol Hydrochloride also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for propranolol: your liver uses an enzyme called CYP1A2 to help break it down. In lab and animal tests, danshen seems to slow that enzyme down. If that happened in a real person, propranolol could build up a bit more than expected, which might mean a stronger effect, like a slower heart rate, lower blood pressure, sleepiness, or feeling faint.

The honest truth, though, is that this is mostly theoretical. When researchers tested danshen with another CYP1A2 medicine in actual people, they didn't see a real change. Danshen may also nudge blood clotting, which is worth keeping in mind. With Hemangeol being given to a tiny infant, please loop in your pediatrician or pharmacist before combining anything.

What Is Propranolol (Infantile Hemangioma) Used For?

Brand name: Hemangeol

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
