Ethinyl Estradiol, Norgestimate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for your birth control pill (the ethinyl estradiol and norgestimate combo). Danshen has been shown in test-tube studies to slow down a liver enzyme called CYP1A2, which helps clear estrogen from your body. In theory, if that enzyme is blocked, the estrogen part could hang around a little longer, which might bump up estrogen-related side effects like nausea, breast tenderness, headaches, or bloating.

That said, I want to keep this in perspective. This is based only on lab work, not real people, and when researchers tested Danshen with a similar CYP1A2 drug in humans, they did not see a meaningful effect. So this is a theoretical concern rather than a proven problem. Danshen can also affect blood clotting, so it's worth a quick chat with your pharmacist or doctor before combining them.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist with your birth control pill (Ortho Tri-Cyclen and similar brands): your body uses a transporter called P-glycoprotein to help move certain medicines around. Danshen seems to slow that transporter down a bit. In one small study, it raised the blood levels of another drug that relies on this pathway. So in theory, the hormones in your pill (ethinyl estradiol and norgestimate) could build up slightly higher than usual.

If that happened, you might notice more of the usual estrogen-type side effects, like nausea, breast tenderness, or headaches. This evidence is fairly limited and not specific to your pill, so I wouldn't panic. Still, before adding Danshen, please run it by your pharmacist or doctor first.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the practical concern with your birth control pill. The hormones in Sprintec and Ortho Tri-Cyclen, ethinyl estradiol and norgestimate, are processed by a liver and gut enzyme called CYP3A4. When Danshen is taken steadily for a week or two, early research suggests it can speed that enzyme up, which may clear these hormones out faster and lower their levels in your blood.

Why does that matter? If hormone levels drop, the pill may not work as reliably. That can mean breakthrough bleeding or spotting, and in theory a higher chance of an unplanned pregnancy. This is based on limited human data, so it isn't a sure thing, but it's worth taking seriously. Please don't stop either one on your own. Check with your pharmacist or doctor, and consider a backup method like condoms if you use Danshen.
