Why this applies to Perampanel

Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the basic idea with Danshen and Fycompa (perampanel). Your body uses a liver enzyme called CYP1A2 to help clear perampanel. In lab dishes, Danshen seems to slow that enzyme down. If that happened in real life, perampanel could build up a bit, which might mean stronger effects like dizziness, sleepiness, trouble walking, blurry vision, or mood changes.

But I want to be honest about the evidence here. This is only theoretical, based on test-tube studies, and when researchers tested Danshen with a similar CYP1A2 drug in actual people, they didn't see this effect. So real-world risk looks low. Still, since seizure control matters so much, please check with your pharmacist or doctor before adding Danshen.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist with Danshen and Fycompa (perampanel). Your body relies partly on a liver enzyme called CYP2C9 to break perampanel down. Lab studies suggest that compounds in Danshen can slow that enzyme. If it slows down, perampanel may clear more slowly and build up in your blood, which could mean stronger effects and more side effects like dizziness, sleepiness, trouble with balance, or mood and irritability changes.

The honest part: this is only a theoretical concern based on test-tube research, and it hasn't actually been reported in people. So it's not a reason to panic. Still, before adding a Danshen supplement, run it by your pharmacist or neurologist, since seizure meds are worth being careful with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for Perampanel (Fycompa): your body clears this seizure medicine largely through a liver enzyme called CYP3A4. Danshen seems to nudge that enzyme in two different directions depending on how it's used. Taking it steadily for a week or two appears to speed up CYP3A4, which could lower your perampanel levels and, in theory, make your seizure control less reliable. Oddly, a single dose did the opposite in studies, briefly raising drug levels (which could mean more dizziness, sleepiness, or trouble with balance).

This is based on limited research using a different drug (midazolam), so it's a possible concern rather than a proven one. Since steady seizure protection really matters, please check with your pharmacist or doctor before pairing Danshen with Perampanel.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
