CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Estradiol, Norgestimate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist of it. Your hormone product (Ortho-Prefest) contains estradiol, which your body partly clears through a liver enzyme called CYP1A2. In test-tube studies, Danshen seems to slow that enzyme down. If that happened in real life, your estrogen levels could creep up a bit, which might mean more of the typical estrogen-related effects like breast tenderness, nausea, bloating, headaches, or spotting.
The important part: this is theoretical. It comes from lab studies only, and when researchers tested Danshen against a real CYP1A2 medicine in people, they didn't see this effect. So I wouldn't panic. Since Danshen can also affect blood clotting, it's still worth a quick chat with your pharmacist or doctor before combining the two.

P-GLYCOPROTEIN SUBSTRATES

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the situation with your Ortho-Prefest (estradiol and norgestimate). Danshen may slow down a transporter in your body called p-glycoprotein, which helps move certain medicines along and clear them out. In a small study, danshen raised the blood levels of one drug handled this way by about 30 to 40 percent. Your hormones aren't a prodrug, so if danshen does the same thing, you could end up with a bit more hormone hanging around than usual.
In real terms, that might mean more of the typical estrogen-type effects like breast tenderness, nausea, bloating, headaches, or spotting. The evidence here is limited and not certain, but since danshen also affects blood clotting, it's worth a quick chat with your pharmacist or doctor before combining them.
