Acetaminophen, Dichlorophenazone, Isometheptane falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

This combo medicine (often sold as Isocom) pairs acetaminophen for pain with dichloralphenazone, a mild sedative, and isometheptene, which narrows blood vessels to ease migraines. The concern here is that Danshen may, in a test-tube setting, slow down the CYP1A2 enzyme that helps your body process certain drugs. If that happened, levels could creep up, and in theory you might feel extra drowsy or notice more side effects.

The reassuring part: this is only laboratory data. When researchers tested Danshen against a real CYP1A2 drug in people, they did not see a meaningful effect. So this is a theoretical caution, not a proven problem. Still, since Danshen can also thin the blood, it is worth running it by your pharmacist or doctor first.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2E1. So far, this interaction has not been reported in humans.

This combination pill (sold as Isocom) contains acetaminophen for pain, isometheptene to ease blood vessel changes in a migraine, and a mild sedative called dichloralphenazone. The piece worth talking about here is the acetaminophen. Your liver uses several pathways to clear it, and one of them is an enzyme called CYP2E1. Lab studies suggest some compounds in Danshen can slow that enzyme down. In theory, that could let a bit more acetaminophen linger in your system.

I want to be honest that this is only seen in test-tube and animal work, never reported in actual people, so I would not lose sleep over it. Still, since high acetaminophen levels can stress the liver, it is worth mentioning Danshen to your pharmacist or doctor before combining them, especially if you drink alcohol or take other acetaminophen products.

GLUCURONIDATED DRUGS

Acetaminophen, Dichlorophenazone, Isometheptane also falls under GLUCURONIDATED DRUGS.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

This combination product (Isocom) contains acetaminophen, and acetaminophen is cleared from your body partly through a process called glucuronidation. Danshen has been shown in lab and animal studies to tug on those same glucuronidation enzymes, sometimes speeding them up and sometimes slowing them down. Because the lab results point in both directions, we honestly can't say with confidence which way it would go in a real person, and importantly, this has never actually been reported in humans.

So this is a theoretical concern based on test-tube data, not something I'd lose sleep over. Still, since acetaminophen is hard on the liver in large amounts, it's worth a quick chat with your pharmacist or doctor before adding Danshen, especially if you take acetaminophen regularly or have any liver concerns.
