Acetaminophen, Codeine, Doxylamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the short version for Mersyndol, which combines acetaminophen, codeine, and doxylamine. The concern on file is that Danshen may slow down a liver enzyme called CYP1A2. In theory, that could let drugs handled by that enzyme build up a little, possibly making you feel more drowsy or boosting side effects like dizziness, an upset stomach, or constipation.

But I want to be honest about how weak this is. The evidence comes only from lab and test-tube studies, and when researchers actually tested it in people, they did not see a real effect. Codeine is mainly activated by a different enzyme (CYP2D6), so this particular pathway isn't its main one. Still, both Danshen and this medicine can affect bleeding and alertness, so please check with your pharmacist or doctor before combining them.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2E1. So far, this interaction has not been reported in humans.

Here's the practical picture with Mersyndol (acetaminophen, codeine, and doxylamine) and Danshen. The piece that matters most here is the acetaminophen portion. Acetaminophen is partly broken down by a liver enzyme called CYP2E1, and one minor pathway turns a small amount into a toxic byproduct that the liver normally mops up. In lab and animal studies only, Danshen appears to slow CYP2E1, which could in theory shift how acetaminophen is processed.

This is a theoretical concern, and it has not actually been reported in people. The codeine and doxylamine parts go through different routes and aren't really the focus here. I wouldn't be alarmed, but since both touch the liver, it's worth running it past your pharmacist or doctor before combining them, and don't exceed the acetaminophen dose on the label.

Acetaminophen, Codeine, Doxylamine also falls under GLUCURONIDATED DRUGS.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the practical picture with Mersyndol (which combines acetaminophen, codeine, and doxylamine). Your body partly clears all three of these through a process called glucuronidation. In lab dishes, danshen seems to both speed up and slow down those clearing enzymes, so the net effect is genuinely hard to predict. The big caveat: this has only been seen in test tube and animal studies, never reported in actual people.

If danshen did slow that clearance, the acetaminophen and codeine could linger a bit longer, which in theory might mean more drowsiness, constipation, or nausea. Because the evidence is so thin, I wouldn't lose sleep over it, but danshen can also affect bleeding, so please run this combination past your pharmacist or doctor first.
