Omeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist for omeprazole and Danshen. Your body clears omeprazole partly using a liver enzyme called CYP2C9 (and another called CYP2C19). In test-tube studies, compounds in Danshen seem to slow that enzyme down. If that happened in real life, omeprazole could hang around a bit longer and act a little stronger, which might mean side effects like headache, nausea, stomach upset, or gas.

The important part: this is only a theory from lab work, and it has not actually been seen in people. Omeprazole is the active drug itself, not a prodrug, so slowing its breakdown would tend to boost it, not weaken it. Still, the real-world risk looks low. If you'd like to use Danshen, just run it by your pharmacist or doctor first, especially since Danshen can also thin the blood.

Omeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with omeprazole and Danshen. Omeprazole is partly cleared by a liver and gut enzyme called CYP3A4 (it leans more on a different enzyme, CYP2C19, but 3A4 plays a role too). The research we have suggests that taking Danshen regularly for a week or two can speed up CYP3A4, which could lower omeprazole levels a bit and make it work less well. In real life that might show up as heartburn or reflux that isn't as well controlled as before.

The evidence here is early and was studied with a different medicine, so this is more of a theoretical concern than a sure thing. Omeprazole isn't a high-risk drug, but if you take Danshen and notice your reflux creeping back, check in with your pharmacist or doctor before changing anything.

About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
