Why this applies to Omeprazole

Omeprazole falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist with Puravive and your omeprazole. Puravive contains quercetin, which can mildly slow down a liver enzyme called CYP2C9. Omeprazole is partly cleared by that enzyme (along with CYP2C19), so if quercetin slows it down a little, slightly more omeprazole could hang around in your body. Omeprazole is not a prodrug, so this would mean a bit more drug effect, not less.

In practice, omeprazole is already very well tolerated, so a small bump usually isn't dramatic. You might notice mild things like headache, stomach upset, gas, or loose stools. The evidence here is limited and based on a different drug (diclofenac), so this is more theoretical than proven. Still, it's worth running by your pharmacist, especially with concentrated supplement doses.

Omeprazole also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the short version: Puravive contains Panax ginseng, which may nudge a liver enzyme called CYP3A4 either up or down. Omeprazole is partly handled by that enzyme (though it leans more on a different one, CYP2C19), so any effect here is likely to be small. Omeprazole is not a prodrug, so it works as-is to lower stomach acid.

If ginseng slowed that enzyme a bit, omeprazole could linger slightly longer (more chance of headache, gas, or belly upset). If it sped it up, your heartburn relief might feel a touch weaker. Honestly, the evidence is weak and contradictory, so I wouldn't lose sleep over it. Still, mention Puravive to your pharmacist before starting it.

About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
