Diazepam falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist: one ingredient in Puravive is quercetin, which can mildly slow down a liver enzyme called CYP2C9. Your body uses several enzymes to clear diazepam (Valium), and 2C9 plays a part. If that pathway is slowed, a little more diazepam may linger in your system than usual.

In plain terms, that could mean stronger or longer-lasting effects from the diazepam, like extra drowsiness, dizziness, foggy thinking, or feeling unsteady on your feet. The evidence here is fairly modest (it comes from a study on a different 2C9 drug), so this is more of a be aware situation than a red alert. Still, since diazepam already makes many people sleepy, it's worth a quick chat with your pharmacist or doctor before combining the two.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the short version on Puravive and your Valium (diazepam). Puravive contains Panax ginseng, and your liver uses an enzyme called CYP3A4 to help clear diazepam. Ginseng can nudge that enzyme either way, so the research is honestly a bit muddled. If it speeds the enzyme up, your diazepam could clear faster and feel weaker, meaning less relief from anxiety, muscle spasm, or seizures. If it slows the enzyme down, diazepam could build up and leave you more drowsy, foggy, or unsteady on your feet.

The good news is this is rated a minor interaction with weak evidence, so it's unlikely to cause big trouble for most people. Still, since the direction isn't clear, run it by your pharmacist or doctor before starting Puravive, especially if you notice any new sleepiness or change in symptom control.

  • Diazepam is used to relieve anxiety and to control agitation caused by alcohol withdrawal. It is also used along with other medications to control muscle spasms and spasticity caused by certain neurological disorders such as cerebral palsy (condition that causes difficulty with movement and balance), paraplegia (inability to move parts of the body), athetosis (abnormal muscle contractions), and stiff-man syndrome (a rare disorder with muscle rigidity and stiffness). Diazepam is also used along with other medications to control seizures. Diazepam is in a class of medications called benzodiazepines. It works by calming abnormal overactivity in the brain.
