Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Here's the connection worth knowing about. Clarithromycin is an antibiotic that can stretch out something called the QT interval, which is a measure of your heart's electrical timing. Puravive contains Panax ginseng, and some short-term studies suggest ginseng may nudge that same interval a little longer too. Put them together and, in theory, the effects could add up and slightly raise the risk of an irregular heartbeat.

I want to be honest that the evidence here is mixed and fairly weak. One study showed a small change, another with longer use showed none. For most healthy people this likely isn't a big deal, but if you have heart rhythm problems, low potassium or magnesium, or take other QT-affecting medicines, please check with your pharmacist or doctor before combining them.

Clarithromycin also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the thing to know about Puravive and clarithromycin: Puravive contains Panax ginseng, and ginseng can tug on a liver enzyme called CYP3A4. Clarithromycin is partly handled by that same enzyme, so in theory ginseng could nudge how your body processes it. The catch is the research points in two opposite directions (some studies show ginseng slowing this enzyme, others show it speeding it up), so we honestly can't predict which way it would go for you.

The good news is this is rated a minor, mostly theoretical concern with weak evidence. If levels did shift, you might see more stomach upset and altered taste, or in the other direction a less effective course of antibiotic. I wouldn't lose sleep over it, but please run it by your pharmacist or doctor before combining them.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
