Why this applies to Sacubitril, Valsartan

Sacubitril, Valsartan falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist with Entresto (sacubitril/valsartan) and Puravive. One ingredient in Puravive, quercetin, can mildly slow down a liver enzyme called CYP2C9. The valsartan part of Entresto is handled in part by that enzyme, so blocking it could let a bit more valsartan linger in your body. That isn't an activation issue, so this isn't a prodrug situation, it simply means a slightly stronger drug effect is possible.

In plain terms, you might notice more of valsartan's usual effects: low blood pressure, dizziness or lightheadedness (especially when standing), or higher potassium. The evidence here is limited and based mostly on small studies, so this is a possible, not certain, interaction. Still, please check with your pharmacist or doctor before adding Puravive.

Puravive contains quercetin, which has been found to slightly reduce blood pressure in individuals with mild hypertension (https://pubmed.ncbi.nlm.nih.gov/17951477/). There is a theoretical possibility that it could enhance the blood pressure-lowering effects of antihypertensive medications.

Here's the simple version: Puravive includes quercetin, and in one small study quercetin nudged blood pressure down a little in people with mild hypertension. Entresto (sacubitril/valsartan) already lowers blood pressure as part of how it helps your heart, so stacking the two could, in theory, push your numbers down a bit more than expected.

If that happened, you might feel lightheaded, dizzy, or extra tired, especially when standing up quickly. To be clear, this is a theoretical, minor concern based on weak evidence, not something proven with this product. Still, since heart failure medications matter, please check with your pharmacist or doctor before adding Puravive, and let them know if you notice any dizziness.

What Is Valsartan and Sacubitril Used For?

Brand name: Entresto

Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).

It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
