Ivermectin falls under P-Glycoprotein Substrates — the group Puravive interacts with.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for Ivermectin: one of the ingredients in Puravive, quercetin, may block a tiny pump in your gut called P-glycoprotein. Ivermectin rides on that same pump, so if it's slowed down, a bit more of the drug could get absorbed into your blood. Ivermectin isn't a prodrug, so this would mean slightly stronger effects, and possibly more side effects like dizziness, nausea, or sleepiness.

I want to be straight with you, though: this is mostly theoretical and based on small studies with other drugs, and the results were honestly mixed. For a typical single or short course of ivermectin, this is rated minor. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

About Ivermectin

What Is Ivermectin Used For?

Brand names: Stromectol, Mectizan, Sklice, Soolantra

Ivermectin tablets (including Stromectol) are used to treat two parasitic infections: strongyloidiasis (a roundworm infection of the intestinal tract) and onchocerciasis (river blindness, caused by a parasitic worm spread by blackfly bites). These are prescription uses for serious infections.

Topical ivermectin lotion — sold under several names including RID One and Done, Head Lice Treatment Ivermectin Lotion, Scalp Care Lotions, and others — is applied to the scalp to treat head lice. A separate topical ivermectin cream (1%), available as Soolantra and ivermectin cream 1%, is used to treat the inflammatory pimple-like bumps of rosacea.
