Why this applies to dabigatran

dabigatran falls under Anticoagulant/Antiplatelet Drugs — the group Puravive interacts with.

Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's the short version for Pradaxa (dabigatran): it's a blood thinner, and Puravive contains a handful of ingredients (kudzu, Panax ginseng, propolis, and phellodendron) that may have their own mild blood-thinning or anti-platelet activity. Stacked on top of dabigatran, the theoretical worry is a little extra thinning, which could show up as easy bruising, nosebleeds, bleeding gums, or longer bleeding from small cuts.

I want to be honest about the evidence, though. Most of this comes from lab and animal studies, and human research hasn't confirmed a meaningful effect. So I'd call this a possible, not a proven, interaction. Still, because bleeding risk is nothing to gamble with, please loop in your pharmacist or doctor before pairing Puravive with dabigatran. Don't stop or change your dabigatran on your own.

P-Glycoprotein Substrates

dabigatran also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the thing to know about Pradaxa (dabigatran): your gut uses a little pump called P-glycoprotein to push some of it back out, which limits how much actually gets into your bloodstream. One ingredient in Puravive, quercetin, may partly block that pump. In theory, that could let a bit more dabigatran absorb than usual, and since this is a blood thinner, the main worry would be easier bruising or bleeding (think nosebleeds, bleeding gums, or dark stools).

That said, this is rated a minor, mostly theoretical interaction, and the human studies on quercetin and this pump have actually gone both directions. I wouldn't panic, but because bleeding risk is nothing to gamble with, please run Puravive by your pharmacist or doctor before combining the two.

About Dabigatran

What Is Dabigatran Used For?

Brand names: Pradaxa, Pradax

Dabigatran etexilate capsules are approved for adults to reduce the risk of stroke and systemic embolism (blood clots traveling through the body) in non-valvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). Capsules are also used to treat deep vein thrombosis (DVT — a blood clot in a deep vein, usually the leg) and pulmonary embolism (PE — a clot in the lungs), to prevent those clots from coming back, and to prevent DVT and PE in adults who have had hip replacement surgery.

For children, the capsule form is approved for treating VTE (venous thromboembolism — blood clots in veins) and reducing recurrence in patients aged 8 to under 18. Pradaxa Oral Pellets are approved specifically for children aged 3 months to under 12 years for the same treatment and recurrence-prevention purposes.
