Why this applies to Triamcinolone

Triamcinolone falls under P-Glycoprotein Substrates — the group Puravive interacts with.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the short version: Puravive contains quercetin, and quercetin may gently block a transporter in your gut called P-glycoprotein. Triamcinolone is one of the medicines that this pump helps move around, so in theory more of it could get absorbed into your bloodstream. Triamcinolone is an active steroid (not a prodrug), so higher levels would mean a slightly stronger steroid effect.

That said, this is mostly theoretical and rated minor. The research is mixed, with some drugs going up and some going down. If anything did shift, you might notice typical steroid effects like fluid retention, higher blood sugar, mood changes, or trouble sleeping. I wouldn't be alarmed, but please run it by your pharmacist or doctor before combining them, especially if you take steady or long-term triamcinolone.

What Is Triamcinolone Used For?

Triamcinolone is available in many forms — injections, creams, ointments, nasal sprays, dental pastes, and eye injections — each approved for specific uses. Injectable forms given into muscle are used when oral steroids aren't an option, covering severe allergies (like asthma flares, severe skin reactions, drug reactions, and serum sickness), serious skin diseases, certain endocrine (hormonal) disorders, and more.

Injections given directly into a joint (intra-articular) or soft tissue help with conditions like rheumatoid arthritis, gout flares, bursitis, and tendon inflammation. Injections into skin lesions (intralesional) treat conditions such as keloids, alopecia areata, and psoriatic plaques. Triesence (an ophthalmic injection) is specifically indicated for eye conditions like uveitis and temporal arteritis, and for visualization during eye surgery. Topical creams, ointments, lotions, and sprays relieve itching and inflammation from corticosteroid-responsive skin conditions. Dental paste forms (Oralone, Kourzeq) treat painful mouth sores from injury. Nasal sprays temporarily relieve hay fever and upper respiratory allergy symptoms such as runny nose, sneezing, and congestion.
