Why this applies to Clomipramine

Clomipramine falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the short version: Puravive contains quercetin, which can mildly slow down a liver enzyme called CYP2C9. Clomipramine is partly cleared by that same pathway, and it's an active drug (not a prodrug), so slowing its breakdown could let a little more of it linger in your system. That may nudge up the usual clomipramine side effects, things like dry mouth, constipation, drowsiness, dizziness, blurred vision, or a faster heartbeat.

Honestly, the evidence here is on the weaker side, and clomipramine is also broken down by other enzymes, so a big jump isn't guaranteed. Still, since this is a medication you don't want swinging around, I'd loop in your pharmacist or prescriber before adding Puravive rather than stopping or changing anything on your own.

Clomipramine also falls under Cytochrome P450 2D6 (Cyp2D6) Substrates.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

Here's the short version: Puravive contains Panax ginseng, which may very slightly slow down CYP2D6, one of the liver enzymes that helps clear clomipramine from your body. Clomipramine is active as-is (not a prodrug), so if it clears a bit slower, levels could nudge upward. In theory that means a little more chance of the usual tricyclic effects, things like dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat.

But I want to keep this in perspective. The research here is weak, and the enzyme slowdown measured was only around 6 percent, with some studies showing no real effect at all. That's a minor, mostly theoretical concern. Still, since clomipramine needs careful dosing, it's worth a quick chat with your pharmacist or doctor before adding Puravive.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Puravive and your Clomipramine (Anafranil). Puravive contains Panax ginseng, which can tinker with a liver enzyme called CYP3A4 that helps break down clomipramine. The tricky part is that ginseng has gone both ways in studies, sometimes slowing this enzyme down and sometimes speeding it up, so we honestly can't predict which direction it would push your levels.
