Ibuprofen falls under Anticoagulant/Antiplatelet Drugs — the group Puravive interacts with.

Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's the thing to keep in mind with this combo: ibuprofen already has a mild blood-thinning, anti-clotting effect of its own because it slows down how your platelets stick together. Several ingredients in Puravive (like Panax ginseng, propolis, and the berberine in Phellodendron) may nudge things in the same direction, at least in lab studies. Stacked together, the worry is a bit more bruising, nosebleeds, or stomach irritation and bleeding, which ibuprofen can cause on its own.

That said, the human evidence here is weak and the effect is considered mild, so don't panic. Still, if you take ibuprofen often, use blood thinners, or have had stomach ulcers, run this past your pharmacist or doctor before pairing the two.

Ibuprofen also falls under Cytochrome P450 2C9 (Cyp2C9) Substrates.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the simple version. Your liver uses an enzyme called CYP2C9 to break down and clear ibuprofen. Puravive contains quercetin, which can mildly slow that same enzyme down. When the cleanup crew works slower, ibuprofen can hang around a bit longer and build up slightly higher than usual.

For ibuprofen, that could mean a stronger chance of its usual side effects, things like stomach upset, heartburn, an irritated stomach lining, or fluid retention and higher blood pressure. The evidence here is on the weaker side (mostly based on a related drug, diclofenac), so this isn't a guaranteed problem. Still, if you take ibuprofen often or have stomach or kidney concerns, it's worth a quick chat with your pharmacist or doctor before pairing them.

What Is Ibuprofen Used For?

Brand names: Motrin, Children's Motrin, Advil, Children's Advil, Fenbid Forte Gel, Ibugel Forte, Arthrofen, Ebufac, Rimafen, Calprofen, Fenpaed, Galprofen, Nurofen for Children, Orbifen for Children, Brufen, Brufen Retard, Fenbid, Tab-Profen, Ibuprofen, Calodor

Oral ibuprofen products (tablets, coated tablets, and liquid-filled capsules) are used to temporarily relieve minor aches and pains from headache, backache, toothache, muscular aches, menstrual cramps, minor arthritis pain, and the common cold, and to temporarily reduce fever. The oral prescription tablet (Ibu) is also indicated for the signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods).

Caldolor, an intravenous (IV) form, is approved for adults and children 3 months and older to manage mild to moderate pain, to manage moderate to severe pain alongside opioid pain medicines, and to reduce fever. A separate intravenous solution, Ibuprofen Lysine, is used specifically in premature infants to close a patent ductus arteriosus (PDA) — an abnormal blood vessel connection near the heart — when standard medical management has not worked.
