Puravive and Imipramine

These two interact in 4 distinct ways. Each one is explained separately below.

Why Are There 4 Interactions?

Puravive interacts with whole categories of medication. Imipramine falls into 4 of those categories, so it can interact with Puravive in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

Cytochrome P450 2C9 (Cyp2C9) Substrates

1 of 4

Why this applies to Imipramine

Imipramine falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the short version: Puravive contains quercetin, which can mildly slow down a liver enzyme called CYP2C9. Imipramine is partly handled by this enzyme, so blocking it could let a little more of the drug build up in your system. Imipramine works as-is (it's not a prodrug that needs activating), so more of it generally means stronger effects, not weaker ones.

If levels climb, you might notice more of imipramine's usual side effects, like dry mouth, constipation, drowsiness, blurry vision, dizziness when standing, or a faster heartbeat. The evidence here is on the weaker side and based on a related drug, so this isn't a guaranteed problem. Still, before pairing the two, please check with your pharmacist or doctor so they can keep an eye on how you feel.

Moderate

Qt Interval-Prolonging Drugs

2 of 4

Imipramine also falls under Qt Interval-Prolonging Drugs.

Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Clinical studies indicate that short-term use of Panax ginseng can increase the QT interval (source: https://pubmed.ncbi.nlm.nih.gov/27240158/). However, no changes in the QT interval have been observed with prolonged use (source: https://pubmed.ncbi.nlm.nih.gov/11978148/).

Here's the thing to know about pairing Puravive with imipramine. Imipramine is a tricyclic antidepressant, and one of its known quirks is that it can stretch out the heart's electrical cycle (what doctors call the QT interval). Puravive contains Panax ginseng, which may have a small, similar effect. Put them together and, in theory, you could nudge that QT interval a bit further, which in rare cases raises the chance of an abnormal heart rhythm.

I want to be honest that the evidence here is mixed and not strong. Some short-term studies showed a small QT change with ginseng, while longer use showed none. For most folks this stays theoretical, but if you ever feel a racing or fluttering heartbeat, dizziness, or fainting, treat that seriously. Please run this combo by your pharmacist or doctor first.

Minor

Cytochrome P450 2D6 (Cyp2D6) Substrates

3 of 4

Imipramine also falls under Cytochrome P450 2D6 (Cyp2D6) Substrates.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.
Severity Minor Likelihood Possible Evidence D

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for imipramine (Tofranil). Your body uses an enzyme called CYP2D6 to help clear this antidepressant out of your system. Puravive contains Panax ginseng, and a little research hints that ginseng may slow CYP2D6 down by a tiny amount, roughly 6%. If that happens, imipramine could linger slightly longer, which in theory might nudge up its usual side effects like dry mouth, constipation, drowsiness, blurred vision, or a racing heartbeat.

I want to be honest with you though: the evidence here is weak and studies don't even agree that ginseng affects this enzyme much. A 6% change is small. Imipramine is an active drug (not a prodrug), so this isn't about losing its benefit. Still, since blood levels matter with this medication, just mention Puravive to your pharmacist or doctor before pairing the two.

Cytochrome P450 3A4 (Cyp3A4) Substrates

4 of 4

Imipramine also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the short version: Puravive contains Panax ginseng, and your imipramine (Tofranil) is partly cleared by a liver enzyme called CYP3A4. The catch is that the studies on ginseng pull in opposite directions. In one report it seemed to slow this enzyme, which could let a drug build up. In another it seemed to speed the enzyme up, which could clear a drug faster.

Because nobody can say for sure which way it goes, you can't reliably predict the effect on imipramine. If levels nudge up, you might see more dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat. If levels drop, your depression symptoms could creep back. This is rated a minor, possible, weakly-supported interaction, so don't panic, but please run Puravive past your pharmacist or doctor first.

  • Stress and brain health

Puravive is a branded, multi-ingredient capsule marketed mainly for weight loss by claiming to boost brown fat activity, but there is no published, independent clinical research showing the product itself works for this purpose. Some individual ingredients have been studied in laboratory or small human studies, but those results do not prove the finished blend is safe or effective. Talk to a pharmacist or doctor before trying it, especially if you have health conditions or take medications.

About Imipramine

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
