Dronabinol falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist with Dronabinol (Marinol or Syndros). Your liver uses an enzyme called CYP2C9 to help break this medicine down. Puravive contains quercetin, which can mildly slow that enzyme. If the breakdown slows, a bit more dronabinol may linger in your system, which could nudge up its usual effects, things like drowsiness, dizziness, a racing heart, confusion, or feeling unusually "high" or out of it.

Now, the honest part: this is a possible rather than proven interaction. The clearest data come from a different drug (diclofenac), and culinary amounts of quercetin from food likely won't matter much, while concentrated supplement doses might. Please don't change anything on your own. Just give your pharmacist or doctor a heads-up before pairing these two.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Puravive and dronabinol (Marinol, Syndros). Puravive contains Panax ginseng, and dronabinol is cleared in part by a liver enzyme called CYP3A4. Ginseng has shown mixed effects on this enzyme in studies, sometimes seeming to slow it down and other times speed it up, so the direction of any change isn't clear cut.

If ginseng slowed dronabinol's breakdown, more could build up and you might feel more of its effects, things like drowsiness, dizziness, a racing heartbeat, or feeling 'high' or confused. If it sped things up, dronabinol might not control nausea or boost appetite as well. Honestly, the evidence here is weak and this is rated a minor interaction, so I wouldn't panic. Just mention Puravive to your pharmacist or doctor before combining them.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
