Entrectinib falls under Qt Interval-Prolonging Drugs — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Clinical studies indicate that short-term use of Panax ginseng can increase the QT interval (source: https://pubmed.ncbi.nlm.nih.gov/27240158/). However, no changes in the QT interval have been observed with prolonged use (source: https://pubmed.ncbi.nlm.nih.gov/11978148/).

Here's the concern with pairing Puravive and Entrectinib (Rozlytrek). Puravive contains Panax ginseng, and entrectinib is already known to lengthen the QT interval, which is a measure of your heart's electrical timing. When two things that both stretch that timing are taken together, the effect can add up, and in rare cases that raises the risk of a dangerous heart rhythm.

I want to keep this in perspective. The evidence on ginseng is mixed: some short-term studies showed a small QT increase, while longer use didn't. So this is a possible, not certain, interaction. Still, because entrectinib itself affects heart rhythm, I'd run any Puravive plan past your oncologist or pharmacist first rather than starting it on your own.

Entrectinib also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the gist: your cancer medicine entrectinib (Rozlytrek) is cleared from your body by a liver enzyme called CYP3A4. Puravive contains Panax ginseng, which has a confusing track record with this enzyme. In some studies it seemed to slow CYP3A4 down (which could let entrectinive build up, raising the odds of side effects like dizziness, fatigue, swelling, numbness or tingling, or heart rhythm changes). In others it sped the enzyme up (which could lower drug levels and make it less effective against the cancer).

Because the evidence is weak and contradictory, nobody can say which way it would go for you. With a serious cancer drug, I wouldn't gamble on it. Please check with your oncologist or pharmacist before adding Puravive, and don't change your entrectinib on your own.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
