Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Here's the concern with pairing Puravive and your leukemia medicine Glasdegib (Daurismo). Glasdegib is already known to stretch out the heart's electrical recovery time, something doctors call QT prolongation. Puravive contains Panax ginseng, which some studies suggest can have a small added effect in the same direction. Stacked together, the theoretical worry is a higher chance of an abnormal heart rhythm.

I want to be honest that the evidence here is mixed. Some short-term studies showed ginseng nudged the QT interval, while longer-term use showed no change. Still, because Glasdegib already affects your heart rhythm and your care team may monitor your EKG and electrolytes, I'd hold off on starting Puravive until you check with your oncologist or pharmacist first.

Glasdegib also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Puravive and your leukemia medicine glasdegib (Daurismo). Puravive contains Panax ginseng, which can nudge a liver enzyme called CYP3A4. Glasdegib is cleared by that same enzyme, so in theory ginseng could shift its levels in your blood. The tricky part is that ginseng has gone both ways in studies, sometimes slowing this enzyme and sometimes speeding it up, so we honestly can't predict which direction it would push glasdegib.

If glasdegib were to build up, you might see more of its usual issues like nausea, muscle aches, fatigue, or heart rhythm changes (QT prolongation). If levels dropped, the cancer treatment could work less well. This is a minor, theoretical concern based on weak evidence, not a proven problem. Still, with a serious cancer drug like this, please run any new supplement past your oncologist or pharmacist first.

What Is Glasdegib Used For?

Brand name: Daurismo

Daurismo (glasdegib) is approved to treat newly-diagnosed acute myeloid leukemia (AML) — a fast-growing cancer of the blood and bone marrow. It is used specifically in adults who are 75 years of age or older, or in adults of any age whose other health conditions make intensive induction chemotherapy too risky.

Daurismo is not used alone. It is always given together with low-dose cytarabine, a separate chemotherapy medicine, as part of a 28-day treatment cycle.
