Guaifenesin, Hydrocodone falls under Cytochrome P450 2D6 (Cyp2D6) Substrates — the group Puravive interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about this combo product: the pain piece, hydrocodone, is partly turned into a stronger active form (hydromorphone) by the CYP2D6 enzyme. So hydrocodone acts a bit like a prodrug. The ginseng inside Puravive may very slightly slow down CYP2D6, which in theory could mean a touch less of that conversion, so you might notice slightly less pain relief or cough suppression. The guaifenesin part isn't really affected here.
Honestly, this is a tiny, uncertain effect. Studies showed only about a 6% change, and they don't even all agree it happens. I wouldn't lose sleep over it. Still, if your pain or cough isn't well controlled, mention Puravive to your pharmacist or doctor before changing anything.

Here's the deal with this combo. Puravive contains Panax ginseng, which can mess with a liver enzyme called CYP3A4. The hydrocodone part of your medicine relies on that enzyme. Studies on ginseng have gone both ways: sometimes it seems to slow CYP3A4 down, sometimes it speeds it up. So we honestly can't say with confidence which direction it would push your hydrocodone.
If ginseng slowed clearance, hydrocodone could build up a bit (more drowsiness, dizziness, constipation, or slowed breathing). If it sped things up, your cough and pain relief might feel weaker. The guaifenesin piece isn't really a concern here. This is rated minor and the evidence is weak, but with an opioid I'd still run it by your pharmacist first.
