Naproxen Sodium falls under Anticoagulant/Antiplatelet Drugs — the group Puravive interacts with.

Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's the thing with naproxen (the pain reliever in Midol Extended Relief): it already has a mild blood-thinning, anti-platelet effect of its own, and it can irritate the stomach lining. Several ingredients in Puravive, like Panax ginseng, propolis, kudzu, and berberine from Phellodendron, may also gently nudge platelets toward not clumping as well. Stacking those on top of naproxen could, in theory, make you bruise or bleed a little more easily, or raise the odds of stomach irritation or a GI bleed.

I'll be straight with you though: this is mostly from lab and animal studies, not solid human proof, so the real-world risk looks low for most people. Still, if you take naproxen regularly or have any bleeding or ulcer history, please run Puravive past your pharmacist or doctor first.

Naproxen Sodium also falls under Cytochrome P450 2C9 (Cyp2C9) Substrates.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist: naproxen (the active ingredient in Midol Extended Relief) is cleared from your body partly by a liver enzyme called CYP2C9. Puravive contains quercetin, which can mildly slow that same enzyme down. When the enzyme works slower, naproxen can hang around a bit longer and build up, which may make its typical side effects more noticeable.

For naproxen, that means a higher chance of stomach upset, heartburn, ulcers or stomach bleeding, fluid retention, and raised blood pressure, especially with regular use. The evidence here is modest, based mostly on a small study using a related drug, so this isn't a guaranteed problem. Still, if you plan to take both, check with your pharmacist or doctor first.
