Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. Here's the thing to know about pairing Puravive with your Ivosidenib (Tibsovo). Ivosidenib can affect your heart's electrical timing, something we call QT prolongation. Puravive contains Panax ginseng, and some short-term studies suggest ginseng can lengthen that same interval too. Stacked together, the worry is an additive effect that could, in theory, raise the risk of a dangerous heart rhythm.

I want to be honest about the evidence: it's mixed. Some studies showed a small change, others showed none with longer use, so this is more of a caution than a sure thing. Still, because Ivosidenib already carries a real QT warning, I'd run Puravive past your oncologist or pharmacist first rather than starting it on your own.

Ivosidenib also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the short version: Puravive contains Panax ginseng, and your leukemia or bile duct medicine, ivosidenib (Tibsovo), is cleared by a liver enzyme called CYP3A4. Ginseng has shown mixed effects on that enzyme in studies, sometimes slowing it down and sometimes speeding it up, so we honestly can't predict which way it would push ivosidenib levels. If levels drift up, you could see more side effects like nausea, diarrhea, fatigue, or a dangerous heart rhythm change called QT prolongation. If levels drift down, the medicine may simply work less well against your cancer.

The evidence here is weak and theoretical, but ivosidenib is a serious drug with a narrow safety margin. Please don't start Puravive without first checking with your oncology team or pharmacist.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
