Why this applies to Darolutamide

Darolutamide falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation. Your prostate cancer medication, darolutamide (Nubeqa), is partly cleared by a liver enzyme called CYP3A4. Puravive contains Panax ginseng, which seems to nudge this enzyme in both directions depending on the study. So in theory, ginseng could either slow darolutamide's breakdown (raising its levels, which might mean more fatigue, rash, or aching joints) or speed it up (lowering levels and possibly making it work less well against the cancer).

The honest truth is that this is shaky, theoretical territory. The evidence is weak, the effect is rated minor, and we don't know which way it would tip. Still, because darolutamide is treating something serious, I'd run Puravive past your oncologist or pharmacist before adding it.

P-Glycoprotein Substrates

Darolutamide also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for your prostate cancer pill, darolutamide (Nubeqa). It's carried through your gut by a little pump called P-glycoprotein. Puravive contains quercetin, which may partly block that pump. If that happens, a bit more darolutamide could slip into your bloodstream, which in theory could nudge up side effects like fatigue, rash, achy joints, or pain in your arms and legs.

I want to be honest about how soft this evidence is. It's mostly theoretical and based on small studies with other drugs, not darolutamide itself, and the results have even pointed in opposite directions. So I wouldn't lose sleep over it. Still, since this is a cancer medicine, please run Puravive past your oncologist or pharmacist before adding it.

About Darolutamide

What Is Darolutamide Used For?

Brand name: Nubeqa

Nubeqa (darolutamide) tablets are approved for adult men with two forms of advanced prostate cancer. The first is non-metastatic castration-resistant prostate cancer (nmCRPC) — cancer that keeps growing despite hormone-lowering therapy and has not yet spread to other parts of the body. The second is metastatic castration-sensitive prostate cancer (mCSPC) — cancer that has spread but still responds to hormone-lowering treatment.

For mCSPC, Nubeqa can be used alone or in combination with docetaxel (a chemotherapy medicine). In all cases, patients also receive a gonadotropin-releasing hormone (GnRH) agonist or antagonist — a medicine that lowers testosterone — or have already had surgery to remove the testicles.
