Lesinurad falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist: Puravive contains quercetin, a plant compound that can mildly slow down the liver enzyme called CYP2C9. Your gout medicine lesinurad (Zurampic) happens to lean on that same enzyme to get cleared from your body. So if the enzyme works slower, lesinurad could hang around at slightly higher levels than usual.

What might that look like? Possibly a bit more strain on the kidneys, which is already lesinurad's main safety concern (it carries a warning about kidney problems). The evidence here is fairly limited and based on a related drug, not lesinurad itself, so this is more of a caution than a sure thing. Please check with your pharmacist or doctor before pairing the two, especially since your kidneys need watching on this medicine.

What Is Lesinurad Used For?

  • Lesinurad is used in combination with a xanthine oxidase inhibitor to treat hyperuricemia (high levels of uric acid) in people with gout (sudden attacks of redness, swelling, pain, and heat in one or more joints) whose disease is not controlled with their current medication. Lesinurad is in a class of medications called selective uric acid reabsorption inhibitors. It works by helping the kidneys to remove uric acid from the body.
