Crizotinib falls under Qt Interval-Prolonging Drugs — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Clinical studies indicate that short-term use of Panax ginseng can increase the QT interval (source: https://pubmed.ncbi.nlm.nih.gov/27240158/). However, no changes in the QT interval have been observed with prolonged use (source: https://pubmed.ncbi.nlm.nih.gov/11978148/).

Here's the concern with pairing Puravive and Xalkori (crizotinib). Puravive contains Panax ginseng, and crizotinib is a cancer drug already known to affect your heart's electrical rhythm (what doctors call QT prolongation). Some short-term studies suggest ginseng can lengthen that QT interval a little too, so stacking the two could, in theory, add up and raise the risk of an irregular heartbeat. This isn't about ginseng changing how crizotinib is broken down; it's about both nudging the heart in the same direction.

That said, the evidence on ginseng here is mixed, and longer-term use didn't show the same effect. Still, since crizotinib is serious medication, I'd loop in your oncologist or pharmacist before adding Puravive. Please don't stop your crizotinib on your own.

Cytochrome P450 3A4 (Cyp3A4) Substrates

Crizotinib also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Puravive and Xalkori (crizotinib). Puravive contains Panax ginseng, and your liver uses an enzyme called CYP3A4 to clear crizotinib from your body. Crizotinib is active on its own (not a prodrug), so anything that changes that enzyme can shift your blood levels up or down.

The tricky part is that ginseng has gone both ways in studies, slowing the enzyme in one report and speeding it up in another. If it slowed crizotinib's breakdown, you could see more side effects like nausea, vision changes, swelling, or liver strain. If it sped things up, the drug might work less well. Honestly, the evidence here is weak and uncertain. Since crizotinib is serious cancer therapy, please check with your oncologist or pharmacist before adding Puravive.

P-Glycoprotein Substrates

Crizotinib also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist: Puravive includes quercetin, and crizotinib (Xalkori) is partly pushed out of your gut by a pump called P-glycoprotein. In a few small studies, quercetin appears to slow that pump down. If that happened with crizotinib, a little more of the drug could get absorbed, which in theory might nudge up side effects like nausea, vision changes, swelling, fatigue, or liver and heart-rhythm effects that this cancer drug is already known for.

I want to be honest, though: this is theoretical and based on weak, mixed evidence, and quercetin has actually lowered some drugs' absorption in other studies. Because crizotinib is a serious cancer medicine, don't add or stop Puravive on your own. Please run it by your oncologist or pharmacist first.

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) is used to treat adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for the ALK or ROS1 gene abnormality, as confirmed by an FDA-approved test.

Xalkori is also used to treat pediatric patients aged 1 year and older and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL). Additionally, it treats adult and pediatric patients aged 1 year and older with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Note: Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
