Why this applies to Alogliptin

Alogliptin falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the good news for anyone taking alogliptin (Nesina): this one is pretty low on the worry list. The concern with Puravive comes from its Panax ginseng, which can nudge the CYP3A4 enzyme either faster or slower depending on the study. But alogliptin barely relies on CYP3A4 at all. It's mostly passed out through your kidneys largely unchanged, so a tug on that enzyme isn't likely to swing your blood sugar much in either direction.

This is rated a minor, possible interaction backed by weak, mixed evidence, so I wouldn't lose sleep over it. Still, since Puravive's blend hasn't been independently tested, it's smart to check with your pharmacist or doctor before adding it, especially if you watch your glucose closely.

Puravive contains Panax ginseng. Clinical studies indicate that Panax ginseng may lower blood sugar levels. One study (https://pubmed.ncbi.nlm.nih.gov/25015735/) found that supplementation with Panax ginseng led to a significant decrease in post-meal glucose levels and an increase in post-meal insulin levels compared to a placebo. If you are taking medications that lower blood sugar, it's important to monitor your blood glucose levels closely when using Panax ginseng.

Here's the thing to keep an eye on if you're taking alogliptin (Nesina) and thinking about Puravive. This supplement contains Panax ginseng, and some studies suggest ginseng can nudge blood sugar a little lower on its own. Since alogliptin already works to bring your blood sugar down, stacking the two could add up and pull your numbers lower than expected.

To be fair, this is rated a minor interaction, and alogliptin by itself rarely causes low blood sugar. Still, if you do combine them, watch for shakiness, sweating, dizziness, or feeling unusually tired, and check your glucose more often at first. Don't change your medication on your own. Just give your pharmacist or doctor a heads-up before adding Puravive.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
