Acetaminophen, Caffeine, Dihydrocodeine falls under Cytochrome P450 2D6 (Cyp2D6) Substrates — the group Puravive interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo: the dihydrocodeine in your tablet is a prodrug. Your body uses an enzyme called CYP2D6 to turn it into the stronger painkiller (dihydromorphine) that does most of the pain relief. Puravive contains Panax ginseng, which some research hints may slow CYP2D6 just a tiny bit (around 6 percent in one small study).
Because dihydrocodeine has to be activated, slowing that enzyme could in theory mean a little less pain relief, not more side effects. Honestly though, this is weak, inconsistent evidence and the effect is small, so most people won't notice anything. The acetaminophen and caffeine parts aren't affected here. If your pain control feels off after adding Puravive, mention it to your pharmacist or doctor before changing anything.

Here's the wrinkle with this combination pill. The dihydrocodeine part is actually a prodrug, meaning your body has to convert it into its active painkilling form. CYP3A4 is one of the enzymes involved in handling it. Puravive contains Panax ginseng, which lab and small human studies suggest can either slow down or speed up CYP3A4, and honestly the data points both ways, so nobody can predict the outcome cleanly.
In theory, if ginseng nudged this enzyme around, you might notice slightly more drowsiness, constipation, and nausea, or on the flip side a bit less pain relief. The evidence here is weak and this is rated a minor interaction, so I wouldn't panic. Still, since this medicine contains an opioid, please run it by your pharmacist or doctor before combining.
Stimulant Drugs

Here's the simple version. Your medicine already contains caffeine, which is a stimulant. Puravive includes Panax ginseng, an herb that has shown mild stimulant-like effects in a few studies. Stacking the two could, in theory, nudge that stimulant side a little higher. In real life that might feel like extra jitteriness, a faster heartbeat, trouble sleeping, or feeling wired.
The good news is this interaction is rated minor and unlikely, and the evidence behind it is weak. The ginseng wouldn't change how the dihydrocodeine (the pain reliever) or acetaminophen part works. If you notice you feel restless or your heart races, mention it to your pharmacist or doctor before continuing both together.
