Sofosbuvir, Velpatasvir, Voxilaprevir falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the deal with Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) and Puravive. Puravive contains Panax ginseng, which can tug on a liver enzyme called CYP3A4. The voxilaprevir piece of your Vosevi is partly handled by that enzyme. The tricky part is that ginseng has gone both ways in studies, sometimes slowing this enzyme and sometimes speeding it up, so we honestly can't predict the direction.

In plain terms: there's a small chance your hepatitis C therapy could end up a bit stronger (more chance of nausea, headache, or tiredness) or a bit weaker, which matters because you want full virus-clearing power. The evidence here is weak and the risk is rated minor, but since this is a curative course, I'd run Puravive past your pharmacist or doctor before adding it.

P-Glycoprotein Substrates

Sofosbuvir, Velpatasvir, Voxilaprevir also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

If you're taking Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) to clear hepatitis C, here's the gist. Puravive contains quercetin, which may block a gut transporter called P-glycoprotein. All three drugs in Vosevi rely on that pump, so in theory quercetin could let a bit more of the medicine get absorbed. That might raise drug levels somewhat, possibly nudging up side effects like headache, nausea, tiredness, or diarrhea.

Honestly, this is a theoretical, low-level concern. The evidence is weak and mixed, with some studies showing the opposite effect. Since Vosevi is a planned hepatitis C cure course, I'd run any new supplement past your pharmacist or doctor first rather than risk anything.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
